Description

Jim Riley Outreach is a 30-year-old Christian non-profit organization located in Plano, Texas. For the last 10 years we have been operating two transitional living homes for youth who have aged out of foster care or that have become homeless for other reasons and are facing a challenging. Our mission is to equip these at-risk young adults to cultivate long term change. Our services include housing, life skills, educational and vocational assistance, individual life coaching, and mentors.

Case Manager Job Description

Case Supervision of 6-8 Residents

Conduct weekly meetings with residents to monitor program progress and provide accountability towards goals and financial habits.

Tracking resident progress and goals met to ensure forward progress and meet graduation requirements. 

Monitor and enforce program policies and disciplinary procedures as needed.

Tracking resident perseverance points. 

Providing resident updates to senior staff. 

Coordinating appointments with outside resources and providing transportation if needed 

Making connections to additional resources that will aid in resident success as they arise (Dietician, personal trainer, financial coach, tutor, etc.)

Ensuring residents have basic needs met (clothing, food [for initial month] supplies). 

Closing out file when resident has transitioned out of the program. 

Keeping files and paperwork up to date. 

Supporting resident transitions when they leave the program (graduation or otherwise). 

Administer drug tests (as needed).

Facilitating Thursday night dinners: check with residents about meals, purchasing groceries and helping cook/clean up. 

Facilitate and teach life skills classes as needed. 

Assist in planning celebration parties and writing resident achievement papers.

Other duties as assigned.

Requirement

Desired Candidate Qualifications:

 

  • A bachelor's degree from an accredited college is preferred; psychology, behavioral health or social work. 
  • A mature Christian faith and ability to discuss biblical issues with clarity.
  • Be knowledgeable about working with vulnerable populations and understanding these unique needs.
  • Must have a Texas driver's license and reliable vehicle.
  • Must be able to work with young adults with varying degrees of emotional behavior and related issues. 
  • Superb written and verbal communications skills.
  • Proficiency in Microsoft Office Word, PowerPoint, and Excel (Social Media skill is a plus).
  • Able to pass a criminal background check.

The Case Manager plays an essential role in supporting young individuals through various challenges, acting as a guide and advocate in their journey towards personal growth and stability. This position involves working closely with youth to assess their needs, develop plans to meet those needs, and connect them with appropriate resources and services. By fostering a supportive and understanding relationship, the Case Manager helps empower young people to overcome obstacles and achieve their goals, whether they pertain to education, employment, health, or personal development. Through consistent monitoring and adjustment of plans as necessary, this role ensures that each resident receives the tailored support they need to navigate the complexities of young adulthood. The ideal candidate would be an excellent mediator, capable of resolving conflicts before they escalate, a self-starter and self-motivator who is well organized and uses time efficiently, maintains great flexibility, and strives for excellence.
